Sports Performance International for PT, though depending on the type of PT needed that may not work. Two peds that I've heard good things about are Dr. Kibbe Miller-Crowley at Pediatric Associates of Austin (http://www.pediatricassociates.net) and Dr. Angela McConnell at Children's Medical Group.

Dr Kenneth Hargrave with Pediatric Associates - entire practice is great. Love Dr. Oliver (gal) -obgyn w/Women's Partners in Health if you don't mind having to wait from time to time Dermatologist - Colby Evans & Erin McClung - same practice. Both are good.

When I lived there I used Dr. Seeker(he delivered all 4 of my children, plus did 5 surgeries) Dr. Ari Brown was our pediatrician and her husband Dr. Mark Brown was our ENT!
