Wandering Jew will cause skin irritation, but generally not hotspots. It is probably a flea bit - only takes one to start a chewing/itching cycle.

Jay Bailey, coal tar shampoo should be easy to find. The bonus with coal tar is that if left on for 20 minutes, it will kill fleas. However, in my grooming career, I never found anything, prescription or not, that keeps on working. The relief is usually gotten while it's on the dog and maybe for a few days after. I've tried them all.
