Can you PM me a picture of the form. If it needs a Notary I am a notary. You can also go to https://www.uscis.gov/sites/default/files/files/form/i-9.pdf to get a copy of an I-9 with a list of all the acceptable documentation

Go to KentuckyDivorce.com Its so easy. They send you a pdf, get it notarized on Dixie and then take it downtown to the courthouse. Took me 2 weeks.
