Does anyone know of a place that will do home care a couple times a week for a Medicare patient?

I live in Lowell and am an RN. I work for Residential Home Health. We do not do private duty, but we do Medicare Skilled visits. We offer RN, physical therapy, Occupational therapy, Speech therapy, Social work and an Aide to help with bathing. Medicare will pay for skilled visits by these disciplines as long as there was a change in the patient's condition, or a recent hospital stay, or the primary care doctor feels it's necessary. the Patient would need to require the skills of a Nurse or Theapist, and be considered home bound. That does not mean they can't leave the house, but there has to be a taxing effort to leave home. It does require a Doctor's order to have home care. You can just ask the doctor to order it, they will then fax a request to the company and the company will send a nurse out to get them started. Our fax number is 866-903-4000 If you're interested.
