The neighborhood will vehemently oppose nearly all rebuild proposals. Also the city of Madison will throw up a lot of obstacles to any remodeling project. Expect 10-15 revisions to any plan before you get the neighborhood and city approvals. Part of the cost is hundreds of hours of your time.

Flat Fee Home Builders out of Edgerton did ours. This is our third home we've had built. Had an ok experience with Classic Custom homes, they were rigid on their designs. Olsen homes was just ok. Flat Fee Home Builders was amazing.
We met with many builders and they were far an away the best. They are completely flat fee so any changes to the plans are only the cost of materials. We added a whirlpool tub, upgraded light fixtures, added some different closet materials, etc. All of these had no additional charge other than the difference of material. The other builders I have used charge "Change fees" or "score fees" in addition to a crazy difference cost. Upgrading the light fixtures in my current home from standard to something really fancy only was like $50 more.
They also estimated 4 months from start to move-in and it was ready a few weeks early. They stagger the jobs very well so things move quick.

My friend owns a company called progress builders. He is actually doing the same thing right now not too far from you. He is building a large house on a second lot approved for it really close to Ella's Deli in fact. I'd share the street name but I think that may be against the rules of the sub. If you are interested PM me and I can put you in touch with the owner of the company. He does absolutely amazing work as I have him working on a large commercial build out now myself and I am extremely happy with the attention to detail. I do know that others though have found him somewhat difficult to work with but I have had great experience with him. YMMV
edit; I am actually going over to his place later tonight to discuss some details of my project. I can at least mention this to see his interest as well.

We used Encore Construction for our home (this was our first time building). They were only ok. It seemed like we really had to go over how contractors were doing the work to make sure they things done how we had asked them to do it. They would eventually get it right, but seemed like a hassle to get there. My neighbors used Alterra for their house and they haven't had anything bad to say about them. Good luck with your house!
