Dr. Curtis Story on Toledo Blade in Port Charlotte! His practice is called Story Family Medicine. 941-875-9059. He is amazing and not only takes all the time you need with him but explains everything too. He is the very best in the area!!!!!!!!!!!! He used to be with Millenium years ago but thank God he got far far away from Millenium!!! He is amazing!!!

Don't understand the 50.00 a month charge comment. I have been going to Dr Crouch for years and have never seen that charge.

DR Crouch offers a "membership program" for $50 a month, not accepting patients insurance plans at the moment.
