Lots of good suggestions, I would add probiotics (I like Protexin) and start reading up on whole horse health, you may feel it's an impossibility for you but join one of the barefoot groups, you won't believe the amount of knowledge there and lots of people who have got horses like yours back to health. NAF Pink Powders (food additive) is worth a try, it seems to help them make the best of the nutrition they are getting. Worked with a TB I tried everything with.
