I've heard really good things about apple vacations.com. We went to mexico and did it ourselves it turned out awesome but we were kind of on our own on a lot of stuff like transportation to and from our resort. Some people we met used apple vacations and had all of their travel and transportation lined up and ready for them.

Travel agent. It's free, they book for you, they do all the hassle work. And they know of the best packages. We went through Village Charters to an all inclusive adults only spot in the Mayan Riviera called Secrets Maroma. It was absolutely incredible. Doesn't get nicer.
