You could check [Protea](http://www.proteadiamonds.com/index.html) in Arlington. They are highly rated, got my wedding band there, and might do some custom work.

I've used Dominion Jewelers for a couple purchases in the past year and have nothing but wonderful things to say them. In fact, I found the wedding band I wanted at a different jeweler for about $1300 ish, wrote down the designer/catalog number, took it to Dominion jewelers and then they made it for me for $700 ish.

Burke Jewelers in Reston! I used to work there. Larry, the owner, is really friendly and they do quality work.
