Tammy - for tree removal, we used Hercules Tree. They did a great job, and pricing was very reasonable. 330-696-5638. They cut down / hauled away a 30 year old ash tree (about 45 feet tall), and trimmed three oaks in our backyard.

330-832-9856 This is the number for the osu extension office. They offer a free community service by identifying and recommending treatment options for these issues. You could also contact a service forester with the Ohio division of forestry for similar assistance. 440-564-5883 ext. 2 This is a better option than contacting a general tree service in most cases, whom usually aren't familiar with diagnosing tree diseases and treatment options.
