I randomly asked this same question last week! A friend recommended US Recognition and they did a great job. This was just a one time thing, but my friend works with them all the time through his work. They're in Lakewood if it matters.

I just learned that the Pottery Barn/Pottery Barn Kids at Park Meadows has someone that does machine embroidery. She’s in the store, but not an employee of the store, so I’m pretty certain you can bring whatever you want to be embroidered.
