I use www.labradorcnm.com for my CNM, they send the swabs and envelopes, the postage for my last one was only $5. EIC, you have to use sterile swabs. I actually bought a whole box (100 count) for $6-8 when they first started allowing the swabs. All the dogs I originally had done were with blood since they only accepted it at the time. I've had 3 tested with swabs now, and 2-3 litters done with dew claws.

The owner can have other test results recorded at OFA, however, keep in mind that OFA will only accept results from certain approved Labs for certain tests. For example, OFA will still only accept DNA test results from University of Minnesota. Here's an example of my Boykin that I have had all health tests recorded at OFA. Some were automatic like OFA hips and OFA eye clearance, others I had to pay a fee and submit the separate test result. Hope this helps explain the OFA health database where your health results can be centralized. http://www.offa.org/results.html?num&submit=Begin+Search®istrar&namecontains=N&part=carolinas+maximus&breed%5B0%5D=BY&breedlist=ALL&variety%5B0%5D&sex&birthday_start_month&birthday_start_year&birthday_end_month&birthday_end_year&birthday&rptdte_start_month&rptdte_start_year&rptdte_end_month&rptdte_end_year&rptdte
Go to the site Daren provided. And only that site. All instructions are provided. For EIC go to http://www.vdl.umn.edu/ourservices/canineneuromuscular/eic/home.html And only this site. These are the glad that identified and developed the testing.

Cindy DeCaro Spurlin there is a place called Ucan # 721-7387 call them. One of my neighbors take their cats there & said they were the cheapest place ever to get shots, spaying. Although they have never had to have any other type of surgery but if they are the cheapest in that then most likely in surgery as well. Good luck.
