I'll write this for everyone else: Zigi Shaul in Merkaz haCarmel did replacement lenses for me, with Kupat Holim and Student discount, 500 NIS. That said, I do NOT recommend them. They were rude, expensive, and sneaky about giving me a copy of my prescription afterwards. I DO recommend ordering online. I have had a fantastic experience with Zenni Optical, http://www.zennioptical.com - who do replacement lenses, and full glasses through mail order - I got two full pairs of glasses including lenses for under $30 (under 100 NIS). If you already have a prescription, it's easy to enter it in their system online, and use a pic of yourself to 'fit' glasses to your face. It took only about two weeks to receive them, and I did not have to pay any sort of customs duty.
