Ulpan Beit Haam is good too, and Ulpan Morasha has great reviews. All are much less expensive than Ulpan La-Inyan or Ulpan-Or, from what I know.

Yes, 2 just opened up in the Matnas Philip Leon on Chile St #8. Beginners class Monday and Wednesday from 5-8. Intermediate class Monday and Thursday from 5-8. Cost: 80 nis/ month. (It's subsidized) there are also morning classes but I don't know the schedule. 02-547-3669

You have a number of options. You have rights to do both a public and private Ulpan if you were an Oleh of 2017. You can only take a private Ulpan and be fully reimbursed as long as you start before December 2017. After that you can do a public Ulpan if you choose because it works on a voucher system. I work for Ulpan La-Inyan (www.ulpan.com) so of course I am a bit bias but you could speak to people who have done it.

Beit HaAm is also around 800 nis per month depending on the program. Like Morasha, it is public, reimbursed by Misrad Klita, but quite a few of my classmates were paying privately

My friend took This ulpan:http://www.jerusalemvillage.org/what-we-do/alter-native/ where on top of learning to speak higher levels they do activities only speaking Hebrew also mixing with Israelis. My friend also said he signed up for a book club with them, where they read a Hebrew book together and then discuss it in Hebrew. Sounds really interesting and helpful

Beit HaAm downtown (levels to vav, but higher levels way overcrowded and vav is impossible), Hebrew U/Har Hatzofim (superb but olim benefits don't cover you there), Morasha (only lower levels, but good conversation), Ulpan Mila (downtown, private, excellent and not too unreasonable), Ulpan LaInyan (AACI building, small class sizes are great, Pimsleur method), TalkCafe (all over town including Smadar on Emek Refaim, very small classes are fantastic), ICCY on Emek Refaim (both ulpan-light and classes in easy Hebrew), Ulpan Aviv (this is the only one I've never tried, very pricy but good rep), Ulpan Etzion (excellent but moved to Beit Canada near the Tayelet and only under-30 are eligible). Lots of great private teachers around too, so the priciest ulpanim--with the possible exception of Hebrew U which is truly fantastic if extremely time-consuming--may be less beneficial than hiring someone privately.
