I have a similar set of injuries, TBI, etc. Doctors aren't doing cards anymore because it's legal. The card/registry only gets you a discount anyway, you don't have to pay the taxes on it and that's it.

if it's not too far for you, Concord Hillside Medical Associates is where I go and I have always been happy there. Ive also heard Southboro Medical on Rt 9 in Framingham is good too
