I've been using a sports chiropractor - Dr Eric Brady - in Provo who does miracles.

If you're going to try the orthotics route, go see Brent Ambury at Park City Bootfitters on Iron Horse Drive. Brent is the guy that World Cup ski racers go see, because proper fit and bio-mechanical alignment are critical in a sport where you go 90 mph downhill on ice without brakes. Sometimes he's had to catch in the summer. Leave a message. It will be worth your time and money.
