My son goes for eye exams at Dr. Karasek's office in Prague 6. He speaks English and takes VZP. There is an optician in the the same building if you need (and want) new glasses made so it's all very convenient. You can make an appointment via e-mail as well. Do you need an optometrist (one who measures your eyes and gives you your glasses or contacts prescription) or an ophthalmologist (if you have medical issues with your eyes, one who can prescribe medications etc.)? If the former, then Grand Optical at Chodov can help you. (there's also opticians, who just make glasses)

If assuming you just want an eye checkup for glasses. Then any Fokus Optik in P6 is good. They speak English at the one in Dejvice.

Fokus Optik in Vodickova. They are a shop and won't take insurance, it costs a little but I had a great customer service and the person in charge checked my eyesight like no one before. I strongly recommend it!

Alta Optika http://www.altaoptika.cz/en/ (near Divoká Šárka) has a pretty good setup, and they speak English. Eye appointment runs about 500Kč (which seems a little steep for me, but I have nothing to compare it to here) and make their own lenses/carry a lot of various frames.

I can highly recommend Easy Optic, the opinions are divided, but the service has always been excellent with me. Ask for Martina, but either of the two people there are always extremely kind, helpful and professional.
