Nope, there's a separate return for your health insurance company, and one that goes to CSSZ. I use https://www.onlinepriznani.cz - it does all 3 for you. It's not entirely too late - you need to file it.

According to the law (§ 2 of Zákon č. 586/1992 Sb., o daních z příjmů) if you have stayed in the CZ more than 183 days per year (consecutive or cumulative), you should pay your taxes here.
