I am looking for a good neurologist and possibly neurosurgeon in Northern California (near Sacramento area) I really want to find someone who has experience with chiari....
Lara T. replied:
Bogen is at UCD Davis. Bogen is considered an expert and appears on some chiari expert lists. Ciricillo is at Sutter. Ciricillo did my son's surgery just over 5 years ago. We never saw Bogen, but he reviewed my son's scans as a favor for a friend. We are on our 13 th neurologist. We currently use Candida Brown out of Walnut Creek, but she sees kids. We have had a hard time finding someone who can understand dysautonomia and the long term damage to my son's brainstem that he sustained in the years he was undiagnosed. Dr. Chez is a neurologist at Sutter Sac who understood chiari enough to get us to a neurosurgeon. He like most of the neurologists we have seen want to throw meds at the headaches my son has that tank his dysautonomia. Surgery saved my son's life since his brain stem was compressed. However the 6 years it took to get diagnosed and have surgery left permanent damage including an inability for my son to thermal regulate. Now my pediatrician does most of the work to manage my son's care. Drugs do not help my son. They make things worse. We used Dr. Khan another Sutter neurologist to check for high ICP. We have also had full autonomic testing done at Mayo and Cleveland.

I know it isn't Northern California, but Dr Langston Holly and Ulrich Batzdorf at UCLA are awesome. They really take the time with each patient. I am due to have a revision surgery done by them because my first surgery (by a different NS) failed and he wouldn't even acknowledge my recurring symptoms. He never did an MRI post op. Turns out I had an internal leak. I love my new NS's.
Melie C. replied:
We've all been Jeannette Francis Maxwell. You get diagnosed and then no NL or qualified NS that can help you. It's frustrating. I recently just went through it myself. The advice I would give you is be your own advocate. Start researching Chiari Malformations, start with conquerchiari.com or asap.org. The best thing I ever did was join a FB group called Beyond the Measurement. I learned so much just from reading the posts/comments. I posted my MRI and they gave me their "non professional" opinions. Tell you about other complications that you should be concerned about. They have many files that you can read that are very helpful for example "what a chairi NS expert will look for..." I recently had surgery w one of the experts in NYC. To be honest, most of them are of the east coast as I understand it. I've heard good things about Dr. Theodore in AZ that I mentioned above.
Natalie W. replied:
Dr Seiff is in Las Vegas, but he's the only close neurosurgeon I know of who specializes in Chiari. He will help you work through your insurance.
Mamie B. replied:
jeannette, the decompression surgery is different for everyone. i did not bounce back. i got worse. it is brain surgery, so it is tough. there are some great neurologists in california, try UCLA and of course Standford neuro departments. Dr Batzdorf is at UCLA and im not sure, hear different things every week, if he is even seeing new patients. he should be able to recommend someone. he's one of the best. depending on how far you have to travel, you may find a good neuro locally to you who can be a part of your team before and after surgery if you have it. most cases, not all, but most cases get worse from the pressure the herniation puts on spinal cord, brain stem. its not something you really wnt to put off. you will want help after.
Jenny C. replied:
For a neurologist in Sacramento area I only recommend Dr Riemer at Sutter. The only one to ever listen and actually care and know his shit. For neurosurgery I only recommend Dr Ciricillo. He did both of my surgeries and will do any future ones I might ever need.
