Dr. Calvin Lee in Cupertino. I worked for him and had him take out mine. He is incredible! His partner, Dr Fujimoto is good, too, but if he's in a bad mood, he let's it be known. Lol but go to Dr. Lee. He is in network with Delta Dental but they can bill almost any insurance.
