I go to Lars Mitchell at Kingsley Physical Therapy for my hip - which I get a new, shiny titanium one in early October this year myself. Lars is the best I have ever found. When's your replacement, Jonnie?
David N. replied:
I've seen a couple ones this year after back surgery. Superior PT is the best I've ever had.

Check out Brighton Institute in BH. I went to Carlo for 3 mos a couple of years ago after an ankle injury (they came recommended by my Dr) and they were awesome. Best calf/foot massages ever! I didn't require a lot of different therapies but I know they offer many of those you listed. They also have a wonderful office staff and billing dept.-- so important!!
