My friend has similar issues [but not dementia]. We like Bay Medical Group. Primary is James Eichel, MD but we usually see his Physician's Assistant, Sabra Daly. Sabra is who Dr. Eichel has see his own mother. They're at 1359 Park Avenue [in a Pediatrics office] and are associated with Stanford Hospital. 510-523-8162. Alameda Hospital has a good pulmonary group and a good nephrologist but if they're not in the health plan, we also recommend Donna Tigno, MD [pulmonologist] near Summitt Hosp. in Oakland. PM me for a MD we do NOT recommend!
