I did my Green Card a couple of years ago through Costa & Riccio (http://www.costariccio.com/) and had a great experience. They were very thorough and accompanied me to the interview even though it wasn't necessary. I think I ended up paying a total of $4,000 including processing fees. I will say, it's worth it to pay a lawyer to be with you throughout the process, not just for consultation. USCIS is notorious for refusing an application on a technicality (e.g. typo in a zip code) but not telling you what the mistake you made was and they do not refund your application fees. Having a lawyer made me feel much more relaxed about the whole process!

https://www.uscis.gov/n-400 Instructions are right there and what to fill and where to send it and how much to pay. You then get asked to come in for biometrics and are handed the info packet for the interview with all the questions and answers, you get the date, do the interview and then get a date for the oath and you're done.
