Have they tried supplements? Smartpak has a great supplement form older, stiff and arthritic dogs. Smartcanine Senior. I second the idea for a second opinion. Have had great luck with the folks at Lee's Summit Animal Hospital

Blue Pearl in Overland Park has an acupuncturist, she came and talked to my class, even showed us on one of our school animals, worked great! At mission Medvet, dr Eric Wilson is excellent surgeon! Have your friends apply for CareCredit, if pay off in 6 months no interest charges and can use at human dentist, eye care ect
