I have had a variety of orthopedic issues over the years requiring surgery and PT follow up (microdiscectomy, rotator cuff repairs both sides, etc.). An orthopedic surgeon sent me to Waldron's Peak Physical Therapy and if I need care they have been my go-to ever since. Waldron's Peak is a group practice and highly professional and up to date in their practices.

Kristin at mend. These two of only a handful of PT's in this area who have gone beyond doctoral training and have done a Fellowship, so she is a post-doc PT. Not only incredibly intelligent in how the body works, but also gifted in the use of her hands. She does a LOT of manual therapy, not just by-the-book ice and e-stim. http://www.mendcolorado.com/

cascade physical therapy in Aurora has some amazing PT's that specialize in sports injuries and prevention. I'm in PT school at CU and several of them help out in our classes. It's about a 20-30 min travel from lafayette, but they're amazing people. Also Craig hospital in denver has amazing specialists in spinal cord injury, people go there from all over the country. http://www.csinjuryprevention.com/

Have you looked at CU Center for Sports Med? http://cusportsmedcenter.com/

When I was seriously injured (neck/back) I was treated at http://bouldersportsmedicine.org/. My therapist has once worked as at PT for the Broncos and they have a team approach - so my neurologist, pain mgt specialist and other therapists were on the same integrated plan. I'm sure there are many great PTs locally but depending on your injury you may need more integrated care.
Dale A. replied:

I have an SCI and TBI (related to brain surgery). I was just evaluated at Craig Hospital and I can say that it is completely life-changing. This is all that they do and they have all the modalities to maximize quality of life and performance. Besides PT, OT and Recreation therapists, they also have a modified gym and an advice line that might have answers to your questions. Advice line:800-247-0257;303-789-8508;nursingadvice@craighospital.org. If you ever want to strategize/talk, I'd welcome the conversation with others who understand.
