Here is the form you need to complete. When it's all done you need to take completed form without the dog to the USDA for them to approve. Best to make appt in advance. Also Terminal Four as already suggested can help. Most important is the titer test which can take time to process and following the application process exactly. After 3-4 vets, Dr. Mollie Lawlee made house call for my two dogs in city center. Extremely thorough, competent, and will not do a battery of testing unless she sees fit. I am nuts about my dogs health care and she was a delight to work with! Highly recommend her!054-266-2406

Sad they were moved. Their mother might come back at dark. If you take them to the Biblical Zoo the vets there can try to save them. You can call them and ask. The Biblical Zoo has wildlife rehabilitation. When it gets cold at night they won't be able to regulate their body temperature without their mother to warm them, and might not live.

The Biblical Zoo is the best bet. Wildlife rehabilitation vets often use cats milk for wild animals because they can digest it.

Most vets charge less for street cats. Not sure where there is one in town. Try tza'ar baalei Chaim or if not Dr hasson on Rachel imenu he's an angel 0522600264
