I used to see Dr. Jeffrey Lem. Can't remember if he went to Lowell but thought he did. He has a location in the Crocker Galleria. I now see Dr. James Wong in Japantown. His office is called Wong, Totsubo, Ng and Hom OD.

I really really like Dr. Hirabayashi. His office is right by Union Square. He's a great guy and his optician, Howard is phenomenal for choosing the perfect glasses to fit your face!

Not sure about the insurance part, but I've used Rudy Project for perscription riding glasses the last decade with great results. I've done most of my prescriptions through SportRX.
