Is a Pre-College Physical required at Cal Poly? If so where can I find the paper I need to give to my doctor? Thanks!
Lunchboxzez1229 replied:
There are a couple of immunizations you'll need listed [here.](http://www.hcs.calpoly.edu/content/health/req_immunizations) Most likely you'll already have them all except possibly the Meningococcal Conjugate, which you don't need to get anyway. Other than that, all forms you'll need are [here.](http://www.hcs.calpoly.edu/content/health/forms)
