The one I frequently visit is Presto on the Haarlemmerstraat. You're sure to find a good bike jersey over there. Also make sure to visit the Fietskantine on the Overtoom. I don't know if they sell bike jerseys, but it's a nice place to hang out.

They're located on Westerstraat 216. http://www.vakantiefietser.nl/the-holiday-biker/ These guys have an interesting workshop you might like to visit. It's close to the other one I've mentioned. Their address is Lijnbaansgracht 32B-Hs. http://www.workcycles.com/ I don't know if either of them have shirts.

AFAIK there is no such thing as a famous bike shop here. Cycling is a practical activity here and not some kind of fashion/lifestyle statement as it seems to be in the US. As a consequence, bike shops are very down-to-earth and usually small places here.

My go to if I have any issues while I'm in the city is [Black Bikes Rental](http://black-bikes.com/bike-repair/), they have multiple locations. You'll probably either want the one at Rembrandtplein (Reguliersbreestraat 41, 1017 CM) or 9 streets (Wolvenstraat 18, 1016 EP Amsterdam)

It's Amsterdam. Any shop outside of the Centre (canal area's) is good. This is like asking for a good gun shop in Arizona.

I've used flattire before, and they've been very good. They send someone to wherever you are to make any fixes you need. If it's something major like changing of chains etc, you might want to give them a heads up to make sure they have the necessary equipment with them.

I go to the Sportcentrum VU. They have 3 locations if I am correct. They may not be the cheapest but the people working there are really helpful and help you set up a fitness plan without additional costs.

I practice jiu jitsu at the USC, the University of Amsterdam's sport facilities. Their big sportcentre "Universum" offers pretty much any indoor sport you can think of, including all those you've mentioned. Finding a sublet for july+august shouldn't be too hard, since many students sublet their place during the summer. You could try www.kamernet.nl to find a room.

I've found good gyms to be scarce and expensive. Your Future Fitness Center is one which I liked in my neighborhood, mostly because of proximity though. Suggestion: Ride your bike south along the Amstel out of town. The traffic lights and crossings end quickly after getting past the A-10 and you can really open up the speed. If you can't get your heart rate up within 15 minutes from the center you must be Lance Armstrong or something.

I work out at home using the TacFit Commando system. You can donwload it from... the usual places. After trying it out "for free" for 2 months I actually bought it, that's how impressed I was. It's a one-stop-shop bodyweight exercise regime with everything you need, a video debrief of all the techniques, follow-along-videos, workout calendar charts and nutrition guide. Only thing you need is a computer or phone to watch the follow along videos on and space about the size of a yoga mat (very usable in an Amsterdam apartment) . Also, enjoy Amsterdam :)
